Witness Information

It is important to get sufficient information from each witness so they can be contacted at a later date. The adjuster should verify and record home and work telephone numbers, Social Security numbers, employer’s name, home and work addresses, and the address and telephone number of a person who will always know where the witness is located if he or she should move.
